ASPS Updates Guidance on Gender-Related Surgery for Minors

TL;DR Plastic Surgery Society recommends to delay any gender changing surgery until adulthood.

The American Society of Plastic Surgeons (ASPS) has released an updated position statement clarifying the role of plastic surgeons in the care of children and adolescents experiencing gender dysphoria or gender incongruence.

The update reflects evolving clinical practices and a growing body of U.S. and international evidence reviews that highlight significant uncertainty regarding the long-term benefits and potential risks of gender-related medical and surgical interventions in minors. Based on the current evidence, ASPS concluded that there is insufficient data to demonstrate a favorable risk–benefit profile for gender-related breast/chest, genital, or facial surgeries in children and adolescents. As a result, ASPS recommends delaying these surgical procedures until patients are at least 19 years old.

Our practice supports the ASPS position and we do not perform gender-related breast/chest, genital, or facial surgical procedures on children or adolescents.

ASPS continues to emphasize respect for patient dignity, compassionate care, and the ethical responsibility of surgeons to carefully weigh uncertainty, irreversibility, and long-term outcomes in pediatric and adolescent populations. The Society also reaffirms its opposition to the criminalization of medical decision-making and encourages surgeons to review the full position statement for detailed evidence summaries, ethical considerations, informed consent guidance, and medico-legal implications.
